Continuity and outside use

Understand the continuity measure without confusing it with hourly time tracking.

Quick answer: Continuity is a quarterly visit-based measure for a physician's roster. FHO+ Billing does not calculate it. The Outside Use Report can help explain where rostered patients received care, but outside use no longer reduces capitation through the former access bonus.

What the measure counts

  • The denominator counts qualifying visits with at least one in-basket service for the physician's rostered patients.
  • The numerator counts qualifying visits delivered by the rostering physician, the physician's FHO, or another acceptable physician under the contract guidance.
  • Multiple in-basket codes billed at one visit count as one visit.
  • The threshold is 75% in each measurement quarter.

When an adjustment applies

  • OMA guidance describes an adjustment only when both linked measurement quarters fall below 75%.
  • The adjustment is 15% of base capitation paid in the first linked quarter and does not include acuity payments.
  • Monthly reports are provisional because claims can still arrive within the normal stale-date window.

What to do in FHO+ Billing

  • Use the app for hourly category totals, limits, documentation, and claim-preparation reports.
  • Use the Ministry and OMA continuity reports for continuity performance.
  • Do not compare a continuity percentage with hourly-code ratios. They measure different things.

Important notes

  • Continuity definitions and reporting rules can change. Check the dated OMA page before acting on a report.
