Hourly payments
- Q310A through Q313A became available for eligible service dates starting April 1, 2026.

Other payment changes
- The blended fee-for-service premium became 30% for eligible in-basket regular and long-term-care services to enrolled patients.
- Selected in-basket procedures receive a 50% blended fee-for-service premium. Use the Ministry appendix for the current code list.
- The FHO after-hours premium became 50% for the eligible Q012 service list published by the Ministry.
- The enhanced Group Management Leadership Payment added $4 per enrolled patient per fiscal year, prorated daily and capped at $100,000 per group, alongside the existing program.
Programs that ended or changed
- Outside-use deductions and access-bonus payments ended. The Outside Use Report continues as a reference report.
- Comprehensive-care capitation payments ended for FHO physicians, with a transition reconciliation period described by the Ministry.
- Continuity of care moved to a separate performance measure. It is not calculated from FHO+ hourly entries.
