Limits and claims
Document a claimed day
Keep the daily totals and activity summaries needed to support hourly claims.
Quick answer: For each claimed service date, record the total duration in all four categories. For Q312A and Q313A, add a daily summary of activities. Patient-level start and stop times are not required.

What to keep
- The service date.
- Total Q310A duration for the day.
- Total Q311A duration for the day.
- Total Q312A duration and a summary of indirect activities.
- Total Q313A duration and a summary of clinical-administration activities.
What you do not need
- Patient-level start and stop times.
- A separate time amount for every indirect or clinical-administration activity.
- A patient-level list in FHO+ Billing.
- A zero-hour record for a day with no FHO+-related work.
Write a useful activity summary
FHO+ Billing asks for a description when you enter indirect care and provides a separate description field for clinical administration. Use a short daily summary that matches the work. OMA guidance does not require patient-level start and stop times or a separate duration for each activity.
| Category | Example summary |
|---|---|
| Indirect care | Charting, reviewed results and reports, prepared referrals, completed care coordination. |
| Clinical administration | Reviewed diabetes panel, planned preventive-care recalls, updated physician-led EMR workflow. |

Important notes
- Only the physician's time counts. Record actual work, not scheduled time.
- The Ministry may request records or other information to support billed work.
