Common situations
Learners and locums
Apply the hourly guidance when teaching during care or covering an FHO practice as a locum.
Quick answer: Eligible teaching tied to care for rostered patients can be billed once for the physician's time, even with several learners. A properly registered FHO locum can bill with the locum's solo billing number and the FHO group number.

Teaching and supervision
- Clinical teaching performed concurrently with direct patient care can be included in direct care.
- Patient-specific review with a learner that arises from direct care can qualify as indirect care.
- Lecture preparation and teaching unrelated to clinical care for rostered patients do not qualify.
- The FHO physician can claim the eligible hour once, even when supervising several learners.
- Work performed by a learner without the physician's direct supervision is not the physician's billable time. Separate follow-up work personally completed by the physician may be reviewed under indirect care.
Locum billing
- The locum must be registered through the proper FHO channels.
- The claim uses the locum's OHIP solo billing number and the FHO's four-digit group number.
- During the first year, hourly-claim payment goes to the FHO group account.
- The group and locum should agree on how hourly payments will be handled and reconciled.
Important notes
- Do not build a teaching or locum preset until the FHO has confirmed how the specific arrangement will be documented and billed.
